Counsel Health is building the most advanced AI-enabled, physician-supervised virtual care company, leveraging access to providers, knowledge, personalization, and, of course, medical AI to create a world where everyone has their own doctor for life. This team is reimagining what care can become when your doctor is always available, infinitely knowledgeable, and deeply personal as intelligence becomes abundant. 

This story starts with a practitioner on the frontlines. Physician and AI researcher, Dr. Muthu Alagappan, practiced medicine in the hallways of Mass General on the East Coast and then UCSF out West, and recognized the growing chasm between clinical demand and provider capacity. Later, he served as the Chief Medical Officer at Notable Health. Notable helps successfully streamline healthcare operations, helping to expand the capacity of administrative staff. From this experience, Dr. Alagappan realized the dire need to expand clinical capacity as well. Counsel was born as a new care delivery platform to extend clinical reach by pairing medical AI with expert physicians.

Put simply, we need new technology to keep up with our growing and aging population. In 2023, the U.S. had 86 primary care physicians per 100,000 population (src). How many patients can a doctor even see in an hour? Maybe 5? The math ain’t math’ing! And we’re not even talking about provider burnout or the financial challenges of becoming a doctor today.

Luckily, consumers (i.e. patients) are getting more comfortable asking ChatGPT and other AI tools medical questions to get the directional answers they need faster than they can see a doctor. AI’s rapid consumer adoption gives us the opportunity to flip the care model on its head. Doctors can supervise and adjust care, instead of authoring it, and multiply clinical capacity to cover the widening gap.

Let’s dive into Counsel’s care delivery. First, their chat interface allows patients to get advice, get care, submit lab orders, request prescriptions, and receive health result reviews, 24/7 asynchronously. Patients can chat with Counsel AI, and then add a human doctor to the chat with just one click.AI handles all of the lower-acuity concerns and triage. When escalation is needed, Counsel taps their provider network of in-house doctors licensed across all 50 states. 

Most importantly, they offer a full continuum of care where users can chat with a physician about any medical concerns, get treatment, and if a prescription is required, this can be fulfilled right in the platform. All handled within minutes during holidays or on weekends. Now we’re talking!

Patients can also upload images, PDFs, or even blurry screenshots of bloodwork, imaging, or MRIs for analysis. Counsel has been benchmarked against “HealthBench” datasets and delivers a much lower rate of inappropriate escalations compared to generic LLMs with best-in-class context architecture and model fine-tuning processes. Counsel’s average provider response time is under 5 minutes, significantly outperforming competitors, which can be 30+ minutes. 

That speed is powered by AI workflows conducting prep work behind the scenes: summarizing patient input, organizing history, and queuing the provider with all relevant context. Their team even built an in-house third-party EHR (electronic health record) product from the ground up. Multiple agents triage and summarize conversations in the background, escalating only when necessary. It’s like an Iron Man suit for doctors!

Counsel Health is available to consumers and as a benefit through health plans and employers. Early partners include Nowsta, Notable Health, and Photon Health, with more to be announced in 2026. Counsel saves partners an average of $381 per engaged member annually. That adds up quickly across employer & health plan populations of millions of members

Expect to see Counsel publish more of its AI research in 2026 to cement its position as the leader in responsible medical AI. And with large-scale LLM interfaces like ChatGPT putting restrictions on what their models can answer, it puts an even stronger emphasis on the importance of specialization that startups like Counsel Health can offer. 

After announcing their $25M Series A led by Andreessen Horowitz & Google Ventures, with participation from early Boston-based investor Asymmetric Capital, Counsel Health is hiring across the board. They’re looking for engineers to scale the platform, AI researchers to conduct model training, build guardrails & enhance safety, and GTM talent from their Boston & New York hubs.

As we head into 2026, look for their team to complete their expansion across all 50 states, publicize their benchmark research, grow a large employer and health plan footprint, and complete platform-level integrations with labs, imaging & pharmacy partners. Oh and support more condition areas to encompass chronic care, primary care, etc.
